The instructions below may be given directly to your broker. Please authorize your broker to release your name as the donor of the stock to Fidelity Investments.
Before You Transfer Your Gift
To avoid confusion, please contact Abigail Sams, Development Executive Assistant, before the gift is transferred and provide the following information in writing, if available:
- Donor’s name and address
- Purpose of the gift, including any restrictions or fund designations
(please indicate if this is a payment on an existing pledge) - Name, location, and telephone number of the broker/agent sending the stock
- Name and symbol of the stock to be transferred*
- Number of shares to be transferred
- Anticipated date of transfer
Gift Valuation
For IRS and gift crediting purposes, the value of your gift is the mean market value of the high and low on the date of transfer.
*Please note: the date of transfer is the date the stock enters Every Child Pediatrics’ account — not the date your broker is instructed.
Special Gift Types
For gifts of stock to a Pooled Income Fund, Charitable Gift Annuity, or Charitable Remainder Trust, please contact our team for special transfer instructions.
* For gifts of mutual funds, please consult with our team in advance to confirm the fund is eligible for transfer.
