Our Care Navigation team provides essential resources for Every Child Pediatrics families, and their work was recently highlighted in a journal published by ZERO TO THREE.
The ZERO TO THREE organization strives to provide babies and toddlers with a strong start to life, and their paper “Closing the Gap Through Enhanced, Family-Centered Approaches to Care Navigation” focuses on the impact of routinely screening young children for developmental delays. This screening is important, as early identification of developmental delays can help patients and families gain access to the intervention services they need. The first few years of a child’s life are crucial, and interventions during this time are most likely to improve developmental outcomes. Despite this understanding, there is often a significant gap between the number of children who receive abnormal results and those who actually receive additional services. The journal delves into these challenges, and how Care Navigation programs are already helping improve referrals and follow-up for patients.
Over the last 5 years, Every Child Pediatrics has worked with the Early Childhood Partnership of Adams County (ECPAC) and Colorado Project LAUNCH to develop and enhance our Care Navigation services. The journal highlights these efforts and the steps our Care Navigators take following a developmental screening. This includes steps such as participating in a provider visit to discuss any “red flags” and following up with families to discuss any questions or address. This information has helped demonstrate that Care Navigation services are key in promoting best practices for screening and referral.
